Evidence Pack Checklist — Template
Purpose
- Track evidence items that support matrix High‑Risk rows when vendor/third‑party documents are provided at submittals.
- Every Partially Compliant row must reference at least one Evidence Pack item by filename.

Usage
- Attach PDFs alongside manifests and update Status to Attached when included in the packet.
- If filenames are known but not yet attached, set Status to Parked.
- Mirror filenames and status in Attachments Manifest and write Item_ID into CSV Decision_Notes for corresponding rows.
- Do not mark a requirement as evidenced by a referenced document unless the actual file is attached; otherwise raise an RFI and keep Missing_Source: true.
