Purpose

• Provide a per‑row mapping of High‑Risk matrix items to exact source file paths and line ranges for auditability.

Instructions

• One entry per High‑Risk Req_ID.
• Capture: Source_Doc, File_Path (relative), Section_Title, Page/Line_Range, Offer_Evidence_Doc + Page/Line, Notes.
• Use repo‑relative paths only (e.g., Reference Examples/Q‑XXXX/…); avoid legacy example-documentation paths.
• If source doc is missing in the repo, keep the row and note “Missing_Source: true; RFI: {link}” in Notes.
• If applicable, include Evidence Pack Item_ID in Notes (e.g., “EP‑001”).
